In 2018 we launched our ambitious 5 year strategy, 'Much More Than a Building', where we proudly commit to delivering the services and care that matter to you. As part of our ongoing commitment we are inviting people to share with us their experiences of Hospice Isle of Man in a short questionnaire titled, 'What matters to you, matters to us'. Your comments are very important to us, so that we can continually improve our services for patients and those who matter to you.
All responses are anonymous and the data gathered will only be used by Hospice Isle of Man for the improvement of our services, because what matters to you, matters to us.
The questionnaire comprises of a series of tick boxes and only takes a few moments to complete. Alternatively you can also complete a hard-copy of the questionnaire which is available from Hospice Isle of Man reception.
We value your comments and thank you in advance for your time.
You can complete your survey online here
Alternatively, if you would like to complete the survey on paper, you can collect a copy from the Hospice reception or download and print a copy by clicking on the link below:
You can post your completed questionnaire to the Scholl Academic Centre Research Team using the following address:

At Hospice, it is our pleasure to care for people with life-limiting illnesses, supporting them to live well, their way, to the end of their life. Because of this we feel it is so important to understand people's experience of end of life care not only through Hospice Isle of Man and our extended services, but within our community.
Sponsored by the Scholl Academic Centre and approved by the Isle of Man Research Ethics Committee, we have launched an End of Life Experience Survey, which aims to improve the delivery of end of life care for all who need it on the Island, and will help us identify groups of people who would benefit from specialist palliative and end of life care input.
If you have recently experienced the end of life care of a loved one, by taking the time to complete our survey your views and opinions could help us and other relevant services on the Island better understand and improve the delivery of end of life care for all who need it on the Island. This includes the experience of carers and families who may not necessarily have received Hospice care.
You can complete your form below, and we thank you in advance for taking the time to help us continue helping our community.
