Integrated Palliative Care Outcome System (IPOS)

This assessment form will be posted to you for you to complete. We will interact with you to identify how you are currently managing, before looking at any struggles you are currently experiencing. We will send you an IPOS form to complete. This will allow us to focus on what is important to you and how we can assist and provide the best support. Care is always led by your priorities, goals and preferences.

Get up and go test

 We will then discuss the next steps with you. This will include the "Get up and go test" which should only be conducted having spoken with a member of our team.
