Data Subject Access Request form

Under the EU General Data Protection Regulations (GDPR), which came into effect on the Isle of Man in August 2018, you have the right to request access to personal data that Hospice Isle of Man holds about you. Whilst you are NOT required (under the Regulations) to complete a form – or indeed to use any particular format - we suggest that it may be useful to use this form to avoid any misunderstanding or misinterpretation.

• You may need to provide evidence of identity to enable the processing to start.
• Your request will be processed without undue delay and in any event within one month of the date of your request.
• If the information contains details of another person, we may need to seek their consent before we can provide that information to you.
• You should also be mindful that you do not have the right to all information and/or documents – but only the personal data identifiable from within them.
