Service improvement questionnaire

In 2018 we launched our ambitious 5 year strategy, 'Much More Than a Building', where we proudly commit to delivering the services and care that matter to you. As part of our ongoing commitment we are inviting people to share with us their experiences of Hospice Isle of Man in a short questionnaire titled, 'What matters to you, matters to us'. Your comments are very important to us, so that we can continually improve our services for patients and those who matter to you.

 

All responses are anonymous and the data gathered will only be used by Hospice Isle of Man for the improvement of our services, because what matters to you, matters to us.

 

The questionnaire comprises of a series of tick boxes and only takes a few moments to complete. Alternatively you can also complete a hard-copy of the questionnaire which is available from Hospice Isle of Man reception.
